What facilities are provided by Hunters Moor?
Hunters Moor Neurorehabilitation has facilities for clients requiring physical and behavioural rehabilitation. There is a highly skilled, dedicated and experienced interdisciplinary team of staff who provide individualised treatment programmes for each client. The team consists of Consultants in rehabilitation medicine, specialist rehabilitation and psychiatric nurses, clinical neuropsychologist, physiotherapists, occupational therapists, speech and language therapists, psychiatrists, podiatrist and dietician. We offer a patient specific services which can range from an assessment of their needs, short-term intensive treatment, respite or long-term slow stream rehabilitation.
Who can be treated by Hunters Moor?
Any person over the age of 16 who has suffered a neurological injury or disorder. The symptoms may include any or all of the following: a physical disability, swallowing and communication difficulties, cognitive impairment and behavioural problems. A variety of different conditions and diagnoses can be treated, including traumatic brain injury, stroke, multiple sclerosis and spinal cord injury. We specialise in managing people with complex needs and are able to accept clients with varying levels of dependence from those in low awareness/minimally conscious state to those who are independently mobile.
What to expect when first referred
An assessment from one or two members of our team will be set up to see if the individual would benefit from a period in the unit. We are looking at this stage for what goals a client and their family would like to achieve and if we have the skills to help. We can also arrange a pre-admission visit so that the client and carers can see the facilities we offer. If everyone agrees that a client would benefit from our treatment, we will arrange an admission date.
How long do clients stay in the residential treatment unit?
The length of stay will vary according to the aims of the admission. Clients are able to stay until their rehabilitation potential is reached and a permanent home is found for them. These decisions will be made following discussions with and the involvement of the individual and their family.
What treatment do clients receive?
They will receive intensive individualised treatment from all relevant members of the team. A goal orientated approach is used to ensure all team members are working together on appropriate goals which relate to the aims of each individual. These aims may relate to their physical progress, functional abilities and independence, leisure and work activities, interaction and communication with those around them and re-integration into the community.
How are family/carers involved?
A ‘key worker’ is assigned to each client. Their role is to facilitate communication and links with the family and carers and regularly keep them updated about the client’s progress and involve them in the goal setting process if possible. We encourage communication and involvement with families and support networks and value their feedback.
Where do clients go after discharge from the residential unit?
Patients will be re-settled at home whenever possible. Sometimes returning to their home is not feasible and they may be re-settled in adapted, supported living with appropriate care. Some clients may require permanent placement in a nursing home.
